Waiver/Release form: I give my permission for above child to be a camper at Miracle Hills Ranch, Bethany, MO. Should any emergency occur, I give my permission to medical staff to treat my child if needed. I release Miracle Hills Ranch, it’s employees, volunteers & staff from and of all and any claims, demands, expenses, causes of actions, law damages & liabilities arising from or in anyway related to the above camper’s participation of Camp Hope. I fully agree to this waiver. I also give Miracle Hills Ranch permission to use my child in photos, videos, etc. for promotional use in Camp Hope, Miracle Hills Ranch & it’s functions.
